Тиурекс® it is shown for treatment of arterial hypertension, swell warm, hepatic or renal origin, hypostases of hepatic origin, most often together with kaliysberegayushchy diuretics.

Structure

Active ingredient: hydrochlorothiazide (hydrochlorothiazide).

: 12.5 mg or 25 mg or 50 mg of a hydrochlorothiazide.

Excipients: lactose, cellulose microcrystalline, hydroxypropyl cellulose, ferrous oxide red (E172), ferrous oxide yellow (E172), magnesium stearate.

Contraindication

• Hypersensitivity to GHTZ, other tiazid, to streptocides or to any other component of drug.
• Renal failure of heavy degree (the clearance of creatinine less than 30 ml/min. and/or creatinine of blood serum is higher than 1.8 mg / 100 ml).
• Anury.
• Acute glomerulonephritis.
• Hepatic coma or prekoma.
• Refractory hypopotassemia, hyponatremia or hypercalcemia.
• Hypovolemia.

• Arterial hypertension of pregnant women.

Dose of medicament is selected and adjusted by

individually. To take a pill inside, washing down with water, after a meal. The daily dose can be divided into one or two receptions.

Initial dose makes 12.5 or 25 mg of 1 times a day. The maintenance dose makes 12.5 mg of 1 times a day.

Before use of medicament in a combination with APF, BRA inhibitors or direct inhibitors of renin needs to correct a hyponatremia or dehydration. Otherwise treatment should be begun under observation of the doctor.

Considering pharmacological effects of GHTZ, it is possible to assume that use of medicine during the second and third trimesters of pregnancy can reduce fetoplacental blood circulation and cause in a fruit or the newborn child has such phenomena as jaundice, an electrolytic imbalance and thrombocytopenia.

Drivers

Treatment of the increased arterial blood pressure of GHTZ demands regular medical observation. The ability to run motor transport and other mechanisms can worsen in case of individual reactions. In particular it concerns the initial stage of treatment, increase in a dose of GHTZ, its replacement and also combined use with alcohol.

Overdose

Symptoms.

Clinical picture of acute or chronic overdose of GHTZ depends on degree of dehydration and loss of electrolytes. At considerable loss of liquid and sodium the overdose can be shown by thirst, weakness, dizziness, vomiting, muscular pain and muscular spasms (for example, spasms of gastrocnemius muscles), the headache, tachycardia, orthostatic hypotension caused by dehydration and a hypovolemia brings to haemo concentration, court, drowsiness, a lethargy, confusion of consciousness, vascular collapse and an acute renal failure. The electrolytic imbalance can lead to developing of cardiac arrhythmias.

Hypopotassemia can be shown by fatigue, muscle weakness, paresthesias, paresis, apathy, a meteorism, constipations or to lead to cardiac arrhythmia. The heavy hypopotassemia can result in paralytic intestinal impassability and disturbance of consciousness up to a hypoglycemic coma.

Treatment.

in case of symptoms of overdose administration of medicament should be stopped immediately. In all cases of overdose of medicament it is necessary to hold the general supporting events. Treatment includes recovery of water and electrolytic balance and, in case of vascular collapse, - transfer to intensive care unit, if necessary - antishock therapy.

Side effects

In the post-marketing period it was reported about such side reactions after use of the medicines containing GHTZ: aplastic anemia, glaucoma, multiformny erythema, muscular spasms, acute renal failure, renal failure, fever, asthenia, NMRS (BKK and PKK).

Other medicaments enhancing antihypertensive effect of GHTZ.

Antihypertensive effect of GHTZ other diuretics, antihypertensive drugs, guanetidin, Methyldopum, antagonists of calcium can strengthen, APF (APF) inhibitors, the blockers of receptors of angiotensin (BRA), direct inhibitors of renin, β-blockers, nitrates, barbiturates, derivatives of a fenotiazin, tricyclic antidepressants, vazodilatator and alcohol.
