Tablets of Telpres Plus apply in arterial hypertension. As the combination with the fixed dose medicament of Telpres Plus is used if reception of a telmizartan as monotherapy does not provide due control of arterial blood pressure.

Structure

One tablet contains (active ingredients):

• a telmizartan - 80 mg;
• a hydrochlorothiazide - 12.5 mg.

Excipients: attracts (E 421), povidone, krospovidon, Megluminum, sodium hydroxide, lactoses monohydrate, cellulose microcrystalline, a gipromelloza, sodium krakhmalglikolit (type A), magnesium stearate, ferrous oxide red (E 172).

Contraindication

• hypersensitivity to any of medicament components;
• hypersensitivity to other substances which are sulfonamide derivatives (as the hydrochlorothiazide is derivative sulfonamide);
• pregnancy or planning of pregnancy;
• cholestatic and biliary obstructive disturbances;
• heavy abnormal liver functions;
• anury, heavy renal failures (clearance of creatinine less than 30 ml/minute);
• refractory hypopotassemia / hyponatremia, hypercalcemia;
• feeding by a breast;
• simptomny hyperuricemia (gout);
• children's age (up to 18 years);
• simultaneous use of a telmizartan and aliskirensoderzhashchy products contraindicated sick diabetes or renal failure (SKF <60 ml/minute / 1.73 m ² ).

Route of administration

Telpres Plus should be accepted to patients at whom arterial blood pressure is insufficiently controlled at use separately of a telmizartan. Before transition to reception of a combination with the fixed dose it is necessary to define a dose of each of components individually. According to clinical indications the direct replacement of monotherapy by therapy with the fixed combination is possible.

Symptoms

At overdose of a telmizartan the most significant manifestations were arterial hypotension and tachycardia; also it is reported about bradycardia, dizziness, vomiting, increase in level of creatinine in blood serum and an acute renal failure. Connect decrease in concentration of electrolytes (hypopotassemia, a hypochloraemia) and a hypovolemia owing to an excessive diuresis with overdose of a hydrochlorothiazide. The most common symptoms of overdose are nausea and drowsiness. The hypopotassemia can lead to a muscular spasm and/or exacerbation of cardiac arrhythmia at simultaneous use of glycosides of group of a foxglove or certain antiarrhytmic drugs.

Treatment

Telmizartan does not leave by a hemodialysis. Patients have to be under fixed control and receive symptomatic and maintenance therapy. Therapy depends on time which passed after administration of drug, and weight of symptoms. The recommended actions include stimulation of vomiting and/or gastric lavage. At treatment of overdose it is possible to apply activated carbon. It is necessary to control the level of electrolytes and creatinine in serum. When developing arterial hypotension of the patient it is necessary to put on a back and to help, directed to fast completion of volume of liquid and salt in an organism.
