For completion of a lack of liquid of an organism and in a complex of actions of intensive care; as solvent of other compatible medicines. It can locally be applied to washing of wounds, eyes, a mucous membrane of a nose and also catheters and systems for transfusion.
Contraindication
Overhydratation, a hyperchloremia, a hypernatremia, chloride acidosis, states connected with danger of edematization of lungs.
Drug is not used for washing of eyes at ophthalmologic operations.
Route of administration and doses Appointby
intravenously, rektalno and outwardly. is Entered intravenously by drop infusion to 3 l and more solution in day with a speed of 4-10 ml/kg/h, depending on a clinical situation and level of loss of liquid; apply orally; appoint 75-100 ml in enemas; apply to washing of wounds, eyes, mucous membranes.
Side reactions
are not observed by at the correct use of medicine. When performing massive infusions the development of chloride acidosis is possible.
Overdose
Can lead to chloride acidosis, increase in removal of potassium from an organism, an overhydratation and a hypervolemia owing to what heart failure can develop. At emergence of symptoms of the specified states the introduction of medicine should be stopped, to estimate a condition of the patient and to render the adequate help.
Feature of application
At long introduction, especially in large volumes, control the level of electrolytes in plasma and urine and also a diuresis.
is Carefully entered the patient with violation of secretory function of kidneys, dekompensirovanny heart diseases, at a hydropic and ascitic syndrome at patients with cirrhosis.
Co-administration with corticosteroids or corticotropin demands constant control of level of electrolytes of blood.
In shock states and at losses of blood along with use of solution of 0.9% of Sodium of chloride can carry out hemotransfusions, transfusion of plasma and plasma substitutes.
Use during pregnancy or feeding by a breast. Drug to be used according to indications.
Children. to Children at shock dehydration (without definition of laboratory indicators) 20-30 ml/kg enter. Further the mode of dosing is adjusted depending on laboratory indicators. The general daily dose depends on water and electrolytic balance, etc.
Ability to influence speed of response at control of motor transport or other mechanisms. does not influence.
Interaction with other medicines and other types of interactions.
Drug is compatible to the majority of medicines therefore it is applied to dissolution of various medicines – except for medicines which are not compatible from sodium chloride as solvent.
Pharmacological properties
Pharmacodynamics. 0.9% solution of sodium of chloride normalizes water-salt balance and fills shortage of liquid in a human body which develops at dehydration or owing to accumulation of extracellular liquid in the centers of extensive burns and injuries, at abdominal organs surgeries, peritonitis.
0.9% solution of sodium of chloride improves perfusion of fabrics, increases efficiency of hemotransfusionic actions at massive blood losses and severe forms of shock.
also disintoxication effect owing to short-term increase in volume of liquid, decrease in concentration of toxic products in blood Renders to , activations of a diuresis.
Pharmacokinetics. it is Quickly brought out of the vascular system. Drug keeps in a vascular bed quickly then passes into the interstitial and intracellular sector. In 1 hour in vessels there is about only a half of the entered solution. Very quickly salts and liquid begin to be removed by kidneys, raising a diuresis.
