Pharmacological properties

Pharmacodynamics. karbotsistein influences a gel phase of slime of airways: by a rupture of disulfide bridges of glycoproteins causes fluidifying of excessively viscous secretion of bronchial tubes that promotes removal of a phlegm.

with activation of sialine transferase – enzyme of scyphoid cells of a mucous membrane of bronchial tubes. Karbotsistein normalizes the quantitative ratio of acid and neutral sialomutsin of a bronchial secret, restores its viscosity and elasticity. Stirs up activity of a ciliary epithelium and improves mukotsiliarny clearance. Promotes regeneration of a mucous membrane of airways, normalizes its structure, reduces a hyperplasia of scyphoid cages and, as a result, reduces products of slime. Restores secretion immunological active IgA (specific protection) and the number of sulfhydryl groups of components of slime (nonspecific protection). Renders anti-inflammatory effect due to kininingibiruyushchy activity of sialomutsin that leads to reduction of hypostasis and a bronkhoobstruktion.

Pharmacokinetics. At intake karbotsistein it is quickly soaked up. The C _max active agent in blood plasma is reached in 2 h. Bioavailability low — 10% of the entered dose (owing to intensive metabolism in a GIT and effect of the first passing through a liver). Karbotsistein and his metabolites are allocated generally with kidneys. T _½ makes ≈2 h

Indication

Treatment of symptoms of violations of bronchial secretion and removal of a phlegm, especially at sharp bronchopulmonary diseases, for example at an acute bronchitis; at exacerbations of chronic diseases of a respiratory system.

Use

is Applied inside. mukolik it is recommended to apply in treatment of children aged up to 15 years.

For the accuracy of dosing of syrup applies a measured spoon with divisions.

1 measured spoon filled to a mark of 5 ml contains 100 mg of a karbotsistein.

Children at the age of 2–5 years: 200 mg (10 ml) of a karbotsistein a day in 2 receptions, that is on 1 measured spoon filled before the second division that corresponds to 5 ml, 2 times a day.

Children at the age of 5–15 years: 300 mg (15 ml) of a karbotsistein a day in 3 receptions, that is on 1 measured spoon filled before the second division that corresponds to 5 ml, 3 times a day.

Maximum single dose for children makes 100 mg (5 ml) of a karbotsistein.

8–10 days.

Duration of application of a karbotsistein for children has to be the shortest, no more than 5 days.

Contraindication

Allergic reaction to any of medicine components in the anamnesis (especially on methylparahydroxybenzoate or other salts of parahydroxybenzoate);

a round ulcer of a stomach and duodenum during aggravation;

I a pregnancy trimester, in connection with insufficient amount of data on teratogenic and embriotoksichesky action.

Side effects

digestion violations, nausea, vomiting, a stomach ache. because of existence as a part of parahydroxybenzoate in isolated cases there can be allergic reactions, including a Quincke's disease, an itch and rash on skin (possibly, remote in time). in case of side effects it is recommended to lower a dose or to cancel administration of drug.

Special instructions

Productive cough is a fundamental mechanism of protection of a bronchopulmonary system and per se it is not necessary to suppress. the combination of medicines which modify bronchial secretion, with the means oppressing cough and/or the substances reducing secretion (group of atropine) is irrational.

to disturbance of bronchial passability at babies. At children of the first year of life the possibility of cleaning of airways of a bronchial secret is limited because of age anatomo-physiological features. Any mucolytic agents it is not necessary to apply at babies.

Treatment needs to be reconsidered in case of lack of effect or increase in expressiveness of symptoms of a disease.

Drug contains sucrose therefore patients with hereditary lack of tolerance to glucose, glyukozo-galaktozny malabsorption or sakharozo-isomaltose insufficiency have to avoid administration of drug.

needs to be considered that in 5 ml of 2% of syrup 3.5 g of sucrose contain.

Medicine contains methylparahydroxybenzoate and also dye a yellow decline of FCF (E110) that can be the cause of the allergic reaction (remote in time).

Use during pregnancy and feeding by a breast. During pregnancy (II and III trimester) and feeding by a breast medicament is used after careful assessment of a ratio advantage for woman/risk for a fruit (child) which is defined by the doctor. There are no data on penetration of a karbotsistein into breast milk.

Children. Children can appoint medicament aged from 2 years. Treatment of children should be carried out under observation of the doctor.

Ability to influence speed of response at control of vehicles or work with other mechanisms. Does not influence.

Interaction

during treatment should not apply other antitussives and means suppressing bronchial secretion. increases efficiency glucocorticoid (mutually) and antibacterial therapy.

Overdose

Symptoms: stomach ache, nausea, diarrhea.

Lechenie symptomatic.
