Description
Solution for injections Lidocaine is shown for local anesthesia (terminal, infiltration, conduction) in surgery, ophthalmology, stomatology, otorhinolaryngology; apply as solvent of antibacterial agents of group of cephalosporins.
Structure
Active ingredient: lidocaine;
10 mg contain1 ml of solution lidocaine of a hydrochloride of monohydrate;
Excipients: sodium chloride, sodium hydroxide of 1 M solution, water for injections.
Contraindication
Hypersensitivity to medicament components, other amide mestnoanesteziruyushchy means, existence in the anamnesis of epileptiform spasms on lidocaine, heavy bradycardia, heavy arterial hypotension, cardiogenic shock, severe forms of chronic heart failure (II-III degrees), a sick sinus syndrome, a WPW-syndrome, Adams-Stokes's syndrome, atrioventricular (AV) blockade of II and III degree, a hypovolemia, heavy dysfunctions of livers/kidneys, a porphyria, a myasthenia, retrobulbar introduction by the patient with glaucoma; contraindicated to patients in the first three months after a myocardial infarction with the reduced warm emission of a left ventricle (less than 35% of norm); fibrillation disorders, anticoagulating therapy; infections in the place of an injection; to contraindicated not contact patients; significant depression of function of a left ventricle.
Route of administration
Administration of lidocaine provides preliminary conducting test for determination of individual sensitivity to this medicine.
For terminal anesthesia the adult to grease mucous membranes with medicament in a dose of 2 mg/kg of lidocaine, anesthesia duration – 15-30 minutes.
Maximum dose for adults – 20 ml.

Drug influences function of the central nervous system therefore after use of lidocaine it is not recommended to run vehicles or to work with other mechanisms.
Overdosestrengthening of side reactions Is possible
.
Symptoms: psychomotor excitement, dizziness, the general weakness, a lowering of arterial pressure, a tremor, a disorder of vision, toniko-clonic spasms, a coma, collapse, AV blockade, asphyxia, an apnoea, a depression, drowsiness, uneasiness, a ring in ears. At considerable overdose the consciousness disturbance, respiratory depression, shock, a myocardial infarction is possible. The first symptoms of overdose at healthy volunteers arise at concentration of lidocaine in blood more than 0.006 mg/kg; spasms – at 0.01 mg/kg.
Treatment: termination of administration of drug, oxygenotherapy, anticonvulsants, vasoconstrictors (noradrenaline, phenylephine hydrochloride), cholinolytics. The patient has to be in horizontal position; it is necessary to provide access of fresh air, supply of oxygen and/or performing artificial respiration. Simtoma from the central nervous system are adjusted by use of benzodiazepines/barbiturates of short-term action. If the overdose arises in the course of anesthesia, it is necessary to apply a short-term muscle relaxant. To apply atropine to correction of bradycardia and disturbances of conductivity (0.5-1 mg intravenously), in arterial hypotension – sympathomimetics in a combination with agonists of β-adrenoceptors. In cardiac arrest immediate holding resuscitation actions is shown. In a sharp phase of overdose of lidocaine dialysis is inefficient. There is no specific antidote.
Side effects
- from nervous system: concern, drowsiness, dizziness, a headache, a sleep disorder, confusion of consciousness, a loss of consciousness, up to a coma, disturbances of sensitivity, muscular twitchings, spasms, the motor block, a dysarthtia, a dysphagy; resistant anesthesia, paresis or elegy of the lower extremities and loss of management of a sphincter (for example, syndrome of a horse tail), pricking of skin.
- At use in stomatology: numbness of language and lips.
- Mental disorders: anorexia, irritability, restlessness, hallucinations, depressions; after use of high doses – the excited state, euphoria.
- from organs of sight: a disorder of vision, a nystagmus, a reverse blindness, a diplopia, flashing of "front sights" before eyes, a photophobia, conjunctivitis.
- from organs of hearing and a vestibular mechanism: vertigo, acoustical disturbances, noise or a ring in ears, a hyperacusia.
- from a cardiovascular system: at use in high doses – arrhythmia, bradycardia, delay of conductivity of heart, cross heart block, a stop of warm activity, a peripheral vazodilatation, collapse; tachycardia, increase/lowering of arterial pressure, heartaches, inflows.
- from digestive system: nausea, vomiting.
- from a respiratory system: short wind, rhinitis, oppression or apnoea.
- from the immune system: allergic reactions, including hypostasis, skin reactions, urticaria, an itching, a Quincke's disease, reactions of hypersensitivity, including anaphylactoid reactions (including an acute anaphylaxis), generalized exfoliative dermatitis; oppression of the immune system.
- Others: feeling of heat, cold or numbness of extremities, malignant hyperthermia, hypostases, weakness.
- Local reactions: in the injection site, the feeling of easy burning disappearing with growth of the anesthetizing effect within 1 min., thrombophlebitis, hyperaemia.
Interaction
- Chlorpromazinum, pethidine, bupivacaine, quinidine, Disopyramidum, amitriptyline, Imipraminum, nortriptilin – at the combined use with lidocaine the concentration of the last in blood plasma decreases. Anticonvulsants, barbiturates (Phenytoinum) – at the combined use with lidocaine the acceleration of metabolism of lidocaine in a liver, decrease in concentration in blood, strengthening of cardiodepressive effect is possible

- Isadrinum, a glucagon – at the combined use with lidocaine raises clearance of lidocaine.
- Norepinephrinum, meksiletin – at the combined use with lidocaine decreases clearance of the last (the toxicity amplifies); the hepatic blood stream decreases.
- Acetazoleamide, thiazide and loopback diuretics – at the combined use with lidocaine as a result of creation of a hypopotassemia reduce effect of the last.
