Pharmacological properties

Pharmacodynamics

Levofloxacin — synthetic antibacterial medicine of group of ftorkhinolon, s (-) an enantiomer of racemic mix of medicine of an ofloksatsin.

action Mechanism. Levofloxacin as antibacterial medicine of group of ftorkhinolon affects the DNK-girazy complex and topoisomerase IV.

Ratio pharmacokinetics/pharmacodynamics. Degree of bacterial activity of a levofloksatsin depends on a ratio of the C max to blood plasma or AUC and the minimum inhibiting (oppressing) concentration (MIK).

resistance Mechanism. The main mechanism of resistance is a consequence of a mutation in gyr-A genes. In vitro exists cross resistance between levofloksatsiny and other ftorkhinolona. Thanks to the action mechanism usually there is no cross resistance between levofloksatsiny and other classes of antibacterial agents.

Control points. The MIK limit values of a levofloksatsin recommended by the European committee on testing of antimicrobic sensitivity (EUCAST) separating sensitive microorganisms from organisms it is intermediate sensitive (moderately resistant) and is intermediate sensitive from resistant organisms, are provided to the tab. of 1 testing of MIK (mg/l).

Table 1. MIK clinical limit values of a levofloksatsin, mg/l

Pathogen Sensitive Resistant
Enterobacteriacae ≤1 2
Pseudomonas spp. ≤1 2
Acinetobacter spp. ≤1 2
Staphylococcus spp. ≤1 2
S. pneumoniae 1 ≤2 2
Streptococcus A, B, C, G ≤1 2
H. influenzae 2.3 ≤1 1
M. catarrhalis 3 ≤1 1
Extreme values which are not connected with types 4 ≤1 2

1 Extreme values concern therapy in high doses.

low level of resistance Is possible

2 (MIK of a levofloksatsin — 0.12-0.5 mg/l), but there are no proofs that it has clinical value for H. influenza.

3 Shtama with sizes MIK, above threshold value between sensitive it is also intermediate sensitive (moderately resistant) strains, very rare or about them it was not reported yet. Tests for identification and antimicrobial sensitivity on any it
