Pharmacological properties

Clomifene (2-[4-(2-chloro,-1, 2-difeniletenit) fenoks] - n, n-diyetilamina dihydrocitrate (mix e- and z-isomer)). active ingredient of medicament — the anti-estrogen of nonsteroid structure stimulating an ovulation. the mechanism of action is explained by ability is specific to contact receptors of estrogen in a hypothalamus and ovaries. medicament in low doses strengthens secretion of gonadotropic hormones (prolactin, follicle-stimulating and luteinizing) and stimulates an ovulation. klostilbegit in high doses slows down secretion of gonadotrophins. does not reveal gestagenny and androgenic activity.

After use it is completely soaked up in a GIT. It is metabolized in a liver. It is excreted generally with bile, is subject to enterogepatichesky recirculation, out of an organism it is brought with a stake. Elimination half-life makes 5–7 days.

Indication

For treatment at anovulatory disturbances of a menstrual cycle, including induction of an ovulation at women with an anovulatory cycle, a kiara syndrome — frommelya, a matte syndrome — leventalya, a secondary amenorrhea of a different etiology (including aminoyards after use of contraceptives), oligomenorrheas, a galactorrhoea (not tumoral genesis), an oligospermatism.

Use

in case of regular cyclic bleedings for the 5th day of a cycle.

Scheme I: a daily dose of 50 mg (1 tablet) daily within 5 days, under control of reaction of ovaries by performing clinical and laboratory trials. The ovulation mostly occurs between the 11th and 15th day of a cycle.

Scheme II: this scheme should be applied in case of failure at treatment according to scheme I. It is necessary to take daily doses on 100 mg (2 pill) for 5 days, since 5th day of the following cycle. If therapy did not lead to an ovulation, it is possible to appoint a repeated course (100 mg). In the absence of an ovulation and in this case after a 3-month break it is possible to try to conduct one more 3-cyclic course of treatment. If the ovulation did not take place after that, it is not recommended to repeat treatment.

General dose throughout a cycle should not exceed 750 mg.

Contraindication

Klostilbegit in an allergy to active ingredient and/or other components of drug; during pregnancy; in a liver disease; to an oothecoma; presence of a tumor; depression of function of a hypophysis; uterine bleeding of an unknown etiology; disorder of vision.

Side effects

belong To the side effects arising generally only in case of use of medicament in high doses a headache, dizziness, nausea, sometimes vomiting, a depression, fatigue, feeling of alarm, insomnia, increase in body weight, an abdominal pain, inflows. such symptoms as deterioration in sight, after the termination of treatment disappear. at some women the increase in ovaries is possible, ovaries can sometimes increase to 4–8 cm therefore it is necessary to control basal temperature and in case of a two-phase temperature curve the treatment should be stopped. at long administration of medicament the hair loss is possible. in some cases note the skin rashes which are followed by an itching, allergic dermatitis, a stethalgia, painful periods, urination disturbance. probability of polycarpous pregnancy increases.

Special instructions

before use of tablets definition of function of a liver, a hormonal condition and level of a gonadotrophin at renal excretion. also careful gynecologic inspection is necessary.

Should be controlled also in dynamics function of a liver, the size of ovaries.

can take the Drug only under control of the gynecologist which can lower a dose of medicament or it only to appoint short-term.

After a successful completion of a course of therapy by Klostilbegit the treatment can be appointed progesterone.

In an initiation of treatment Klostilbegitom perhaps temporary deterioration in sight. In this case the control of vehicles and work with other mechanisms are contraindicated.

Each tablet Klostilbegita contains 100 mg of lactose that should be considered when prescribing medicament sick diabetes.

Interaction

Drug does not interact with other medicines. the complex therapy which is carried out according to indications using the klostilbegit and medicaments of gonadotropic hormones is considered rational.

Overdose

Symptoms: nausea, vomiting, inflows, disorder of vision, hypertrophy of ovaries, stomach ache.
