Solution for injections Betaspan is shown at treatment of various endocrine, rheumatic diseases, kollagenoz, dermatological, allergic, ophthalmologic, gastrointestinal, respiratory, hematologic and other diseases which respond to corticosteroid therapy. Corticosteroid hormonal therapy is accompanying to traditional therapy and is not replaceable. This medicine is shown when the fast, intensive effect of a corticosteroid is necessary or desirable. Betaspan is intended for fast and powerful therapeutic effect.
Structure
Active ingredient: betamethasone;
1 ml of solution contains phosphate sodium betamethasone – 5.3 mg in terms of betamethasone of 100% substance – 4 mg;
Excipients: dinatrium edetat, hydrophosphate sodium a dihydrate, the phosphoric acid concentrated, water for injections.
Contraindication
- Hypersensitivity to betamethasone, to other components of medicine or other glucocorticosteroids.
- Peptic ulcer of a stomach and duodenum.
- Acute infectious processes: viral infections and system fungal infections.
- Tropical parasitic infections.
- After vaccination by live attenuated viruses.
Route of administration
Betaspan can be entered intravenously, intramusculary, vnutrisustavno, into places of defeat and also into soft tissues.
Dose and the mode of dosing should be defined individually, depending on features of a disease, weight and efficiency of the carried-out treatment.
Initial dose for adults makesto 8 mg of betamethasone a day. In less hard cases the application in smaller doses is possible. If necessary initial single doses can be increased. The initial dose should be adjusted before obtaining the satisfactory clinical answer. If the clinical result is not achieved through a certain span, it is necessary to cancel Betaspan and to reconsider therapy.
For children usually initial intramuscular dose of betamethasone makes 20–125 mkg/kg of body weight a day. The dosage for children of younger and advanced age should be established by the same principles, as for adults (preferring as accurate observance of the doses specified for age and body weight).
Though Betaspan can be entered in several ways, in emergency situations the intravenous administration is recommended. Intravenously by drop infusion Betaspan to enter from 0.9% solution of sodium of chloride or glucose. Betaspan to add to infusion solution during introduction. Unused solution should be stored in the fridge and to use for day.
an initial dose gradually to reduceAfter achievement of positive clinical effect through certain intervals before achievement of the smallest dose which allows to maintain necessary clinical result. Emergence can demand increase in a dose of Betaspan from the patient of the stressful situations (which are not connected with his disease).
At medicament withdrawal after its prolonged use a dose should be reduced gradually.
PregnantSafety of use of medicine during pregnancy is not established to
Feature of application
by therefore Betaspan it is necessary to apply during this period only when the expected advantage for mother exceeds possible risk for a fruit. Children
At long-term treatment of babies and children needs to be watched their growth and development (considering a possibility of oppression of growth and endogenous producing corticosteroids).
Children receiving immunosuppressive doses of corticosteroids have to avoid contact with patients with chicken pox and measles.
Drivers
Betaspan does not influence the speed of response of the patient at control of motor transport or other mechanisms.
But in isolated cases muscle weakness, spasms, dizziness, a headache, psychoemotional instability, a heavy depression before emergence of obvious psychotic reactions, irritability therefore it is recommended to abstain from control of motor transport or other mechanisms during medicament treatment can arise.
Overdose
Acute overdose of corticosteroids, including betamethasone, does not provide development of vitally dangerous states. Except for very high doses the excessive application of corticosteroids does not lead, in the absence of certain contraindications, to emergence of side effects on condition of lack of such states as diabetes, glaucoma, an active round ulcer and if the patient does not take the digitalis drugs, coumarinic anticoagulants and kaliyvyvodyashchy diuretics.
Treatment. Symptomatic therapy of the complications which resulted from metabolic effects of corticosteroids, the main or accompanying diseases, or owing to medicinal interactions.
needs to provide toan organism with enough liquid and to control the level of electrolytes in blood serum and in urine, paying special attention to balance of sodium and potassium. If necessary it is necessary to restore balance of electrolytes.
Side effects
Frequency of development and expressiveness of by-effects (as well as at all glucocorticoids) depends on a dose and duration of therapy. Usually these phenomena are reversible or are minimized by reduction of a dose that it is more preferable in comparison with medicament withdrawal.
- from a cardiovascular system: stagnant heart failure at the patients inclined to this disease; arterial hypertension.
- from nervous system: dizziness, a headache, spasms, increase in intracranial pressure with hypostasis of disks of optic nerves (brain pseudoneoplasm) usually after completion of treatment, migraine.
- from mentality: euphoria, psychoemotional instability, change of mood, a heavy depression up to emergence of obvious psychotic reactions, in particular at patients with the psychiatric anamnesis, changes of the personality, acrimony, insomnia.
- from organs of sight: a subkapsulyarny back cataract, the increased intraocular pressure, glaucoma, an exophthalmos.
- from an endocrine system: secondary adrenocortical and hypophysial insufficiency (especially during a stress – injuries, surgical intervention, diseases), the lowered tolerance to carbohydrates, a demonstration of latent diabetes, the increased need for insulin and oral hypoglycemic means at the patients sick with diabetes, violation of a menstrual cycle, development of a cushingoid state with a hirsutism, striya and an acne, oppression of growth of a fruit or growth of children.
Interaction
Simultaneous use of phenobarbital, rifampicin, Phenytoinum or ephedrine can accelerate metabolism of corticosteroids that will lead to easing of therapeutic effect.
Overshot effect from application of corticosteroids can be observed byat the patients receiving corticosteroids and estrogen.
Simultaneous application of the corticosteroids and diuretics removing potassium ions can cause a hypopotassemia.
Combined application of corticosteroids with cardiac glycosides can increase the probability of developing of arrhythmias or enhance the toxicity of glycosides associated with a hypopotassemia.
